Supported Treadmill Gait Training: Handrail-Assisted Walking Practice
Definition
Supported treadmill gait training is treadmill-based walking practice using handrails or a body-weight-support harness to reduce balance and strength demands during rehabilitation.
What is supported treadmill gait training?
Supported treadmill gait training uses a treadmill fitted with tall parallel handrails, and sometimes an overhead body-weight-support harness, so a person can practise the walking motion with reduced risk of falling and less demand on their own balance and strength.
This makes it possible to practise walking earlier in recovery, or for longer periods, than would be safe or achievable walking unsupported.
How it works
The handrails give a person something secure to hold and physically assist against, while a body-weight-support harness (where used) can offload some of a person's own weight, reducing the strength and balance needed to take each step. Speed and incline are set low initially and progressed gradually.
As walking ability improves, support is reduced step by step — a lighter handrail hold, less harness support, a faster or less assisted pace — moving a person toward independent, unsupported walking.
What the evidence shows
A Cochrane systematic review of treadmill training and body-weight support for walking after stroke found that people who were already able to walk to some degree (but not those fully dependent on assistance to walk at the start of treatment) benefited most, particularly in walking speed and walking endurance. The review did not find strong evidence that these improvements in speed and endurance persisted long after treatment ended, and noted more research is needed on the specific role of handrail support.
The honest picture is that supported treadmill training is a genuinely useful tool for practising walking safely, particularly for people with some existing walking ability, but it works best as part of a broader, progressive gait-rehabilitation programme rather than a stand-alone treatment.
Where this is used
This is centre-based equipment rather than a portable home device. A physiotherapist uses it as part of a supervised gait-rehabilitation programme, and advises on safe walking practice to continue at home as mobility improves.

A physiotherapist assesses current walking ability and fall risk, and sets a safe speed, incline, and level of handrail or harness support before starting.
