Pneumatic Resistance Training: Air-Pressure Strengthening Machines
Definition
Pneumatic resistance training is strength exercise performed against resistance generated by compressed air rather than a weight stack, giving smooth, low-inertia loading with fine adjustment.
What is pneumatic resistance training?
Pneumatic resistance machines generate resistance using compressed air rather than a stack of weight plates. Our centre uses HUR equipment — including a seated leg press — which is widely used in rehabilitation and senior-fitness settings for this reason.
The exercise itself (for example, pressing with the legs against resistance) is the same familiar movement as a conventional weight machine. What differs is how the resistance is generated and adjusted.
How it works
Air pressure provides resistance that follows the body’s natural strength curve more smoothly than a moving weight stack, and removes the inertia (the "jerk" of starting a heavy stack of plates moving) that a conventional machine has. This makes the load feel more consistent through the movement.
Resistance can also be set in much smaller, more precise increments than typical weight-stack steps. This matters for people who are very weak, deconditioned, or early in rehabilitation, where even a small jump in load can be the difference between a manageable exercise and one that is too hard.
What the evidence shows
Pneumatic machines are a delivery method for progressive resistance training, a treatment with strong general evidence in older adults and rehabilitation (see our Weights & Resistance Training modality for the broader evidence base).
There is also evidence specific to pneumatic machines: a randomised controlled trial of a community-delivered pneumatic-machine programme (Gym Tonic) in adults aged 65 and over found that supervised, twice-weekly sessions over 12 weeks improved lower-limb muscle strength, physical performance (measured with the Short Physical Performance Battery), repeated chair-stand ability, and fast walking speed — with high programme adherence, around 90%.
The honest note is that the core benefit comes from progressive resistance training itself, which can also be delivered with conventional weights or bands; pneumatic machines are one well-evidenced, practical way to deliver it, particularly valued for smooth, precisely gradable loading in frailer or more deconditioned people.
Where this is used
This is centre-based equipment rather than a portable home device. A physiotherapist may use pneumatic machines as part of a supervised centre session, and design a complementary home programme — using resistance bands or light weights — to keep progress going between visits.
Safety

- Take extra care with uncontrolled high blood pressure (avoid breath-holding or straining).
- Respect any post-surgery movement restrictions, and avoid loading an acutely injured area until cleared.
