What tennis elbow and golfer's elbow are
Tennis elbow (lateral epicondylitis, sometimes called lateral epicondylalgia) is an overuse tendinopathy affecting the tendons that extend your wrist — chiefly the extensor carpi radialis brevis — where they attach to the bony bump on the outer elbow. It causes pain and tenderness on the outside of the elbow that typically worsens when you grip, lift, or twist. Tennis elbow is genuinely common. It's estimated to affect around 1-3% of the general adult population, most typically affecting middle-aged adults, with no strong difference in how often it affects men versus women (NCBI/PMC). Each year, roughly 0.3 to 1.1 people per 100 will see a doctor about it for the first time (ACOFP). That compared with 90% for simply waiting it out, and only 68% for a corticosteroid injection. Physiotherapy outperformed the injection on every outcome measure from six weeks onward, and notably, 72% of people who initially responded well to the injection saw their symptoms return later (Bisset et al., BMJ 2006). This matters because it reframes the injection-versus-therapy decision: a quick fix isn't necessarily a lasting one, while a guided exercise programme tends to build durable improvement.
More recent evidence adds important nuance here. A 2024 Cochrane systematic review pooling 23 trials and 1,612 participants compared manual therapy plus prescribed exercise against minimal or no treatment. Treatment success was reported by 57% of the manual therapy/exercise group versus 42% of the minimal-treatment group — but the Cochrane authors rated the certainty of this evidence as low and concluded that manual therapy combined with exercise "may result in little to no difference" in treatment success compared with minimal treatment, since the statistical comparison did not clearly rule out no effect (Cochrane Review, Wallis et al. 2024). The same review also found that adverse events — mostly mild and temporary pain, bruising, or gastrointestinal symptoms — were reported considerably more often in the treatment group (around 31% of participants) than in the minimal-treatment group (around 9%), though this finding was rated very low certainty. Structured physiotherapy tends to speed this process up and make it more reliable: in the Bisset 2006 trial, physiotherapy achieved its 94% success rate by 52 weeks, and was notably faster-acting than a wait-and-see approach in the early weeks of treatment. For golfer's elbow specifically, the NHS notes that it typically takes six to eight weeks of consistent, targeted exercise before people notice a meaningful difference in their pain (NHS Inform Scotland). In practice, most people can expect gradual, steady improvement over three to six months with a consistent, exercise-based physiotherapy programme, with many reaching full resolution by six to twelve months. A smaller number of people with more long-standing tendon changes take longer — which is normal, and not a sign that treatment has failed.

- Stretching. Gentle stretching of the forearm extensor (or flexor-pronator) muscle group helps maintain flexibility and reduce strain on the tendon.
- Manual therapy alongside exercise. Hands-on techniques such as mobilisation with movement can reduce pain more effectively than a placebo version of the same technique in the short term, although how long this benefit lasts beyond active treatment is less certain, and mild side effects such as temporary soreness are relatively common (Cochrane Review).
- Grip and functional strengthening. As pain settles, exercises progress to restore the grip and forearm strength needed for everyday lifting and carrying tasks.
- Load and ergonomic adjustments. Practical advice on modifying repetitive movements at work or home, sometimes paired with a short-term counterforce brace or strap, helps protect the tendon while it heals.
A safety note
Most cases of tennis or golfer's elbow are straightforward to assess and manage. Occasionally, elbow pain that seems to come from overuse is actually referred from the neck — the C6 and C7 nerve roots can cause pain felt at the outer elbow. If pain persists despite appropriate elbow-focused treatment, or if it's accompanied by neck pain, numbness, or weakness spreading down the arm, this points toward assessment of the cervical spine rather than the elbow alone, since treating the elbow in isolation won't resolve pain coming from the neck. Imaging isn't routinely needed unless there's a history of trauma, atypical or persistent symptoms, or signs suggesting a fracture or other more serious cause, and conditions such as rheumatoid arthritis are also worth ruling out in unusual presentations.
